When you travel with Lifeline you must have a medical form on file and whatever permission slips are listed below under the specific event name.

Medical Authorization Form

Every event that requires travel away from Praise also requires a notarized Medical Consent Form. These forms may be dated for just the event in question, to encompass an entire calendar year, or until the teen turns eighteen.  These forms are designed to allow a Lifeline leader to be able to act on the parents behalf in the event of an emergency, while contacting the parent to get them there as quickly as possible.  We hope to never need these forms, but we are still required to have them.
